SELF-PAY RATES
Transparent Care, No Surprises
We welcome self-pay patients and are transparent about our process. Fees vary by service type and appointment length. We are happy to provide a Good Faith Estimate before your appointment as required under the No Surprises Act.
HSA and FSA cards are accepted for all services. Payment is due at the time of service for self-pay patients. We accept all major credit and debit cards.

Your Right to Know What to Expect
Under the No Surprises Act, you have the right to a Good Faith Estimate of expected costs before receiving services. Request one at any time by contacting us.
Cancellation and No-Show Policy
To ensure every patient receives the time and attention they deserve, Cornerstone Behavioral Care has the following cancellation and no-show policy.
Cancellation Policy
Patients are required to provide at least twenty-four (24) hours advance notice if they need to cancel or reschedule an appointment.
- Appointments cancelled with less than 24 hours notice, or missed without prior notice (no-shows), will result in a $75 cancellation fee.
- This fee represents the cost of reserved clinical time and may not be billable to or reimbursed by insurance. The patient is therefore personally responsible for payment of this fee.
- The fee may be charged to the credit card on file or billed directly to the patient’s account.
Medicaid Patients
- Patients covered by Medicaid are not subject to the $75 cancellation or no-show fee, as this charge is prohibited under Medicaid guidelines.
- However, consistent attendance remains essential to providing appropriate care. After three (3) late cancellations or no-shows, the practice reserves the right to discharge the patient from the practice, consistent with applicable medical board guidelines and Medicaid program requirements.
Repeated Late Cancellations or Missed Appointments
Consistent attendance is necessary to provide appropriate medical care and maintain access for all patients.
- There will be a one-time exception for a cancellation within less than 24 hours notice, and the fee will be put in effect for any subsequent cancellations that violate Cornerstone Behavioral Care’s policy.
- After three (3) late cancellations or no-shows, the practice reserves the right, at its sole discretion, to determine whether continuation of the patient-provider relationship is appropriate.
- If the practice determines that it can no longer provide care, the patient may be formally discharged from the practice, consistent with applicable medical board guidelines and practice policies.
- In the event of discharge, the practice will provide written notification and reasonable assistance in transferring care, as required by applicable laws and regulations.
Privacy and Compliance Statement
This policy and all patient information collected by the practice are maintained in accordance with applicable federal and state privacy regulations, including the Health Insurance Portability and Accountability Act (HIPAA). Patient information will be used and disclosed only as permitted by law and in accordance with the practice’s Notice of Privacy Practices, which is available upon request.
